Minimum Requirements
Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA or equivalent accrediting body. Current, active, and unencumbered Oregon Physical Therapist license required prior to start. Current BLS (CPR) certification from AHA or ARC. Home Health experience preferred. Experience providing in-home evaluations, treatment planning, and home environment assessments is preferred. Familiarity with documentation systems and basic computer skills, including email and electronic charting. Ability to supervise and evaluate PTAs and Home Health Aides as assigned. Reliable transportation, valid driver’s license, and required agency liability insurance for home visits. Two to three years of clinical physical therapy experience preferred.

Facility Type: Home Health. Shift Name: Evenings. Shift Pattern: 5x8 schedule. Assignment Type: Travel. Candidate will perform patient evaluations and provide therapeutic services according to physician orders. Role includes supervising and evaluating assigned PTAs and Home Health Aides.
