Minimum Requirements
Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA. Active South Carolina physical therapy license required to start the assignment. BLS (Basic Life Support) certification from AHA or ARC may be required. Home health experience required. Experience with in-home patient care and documentation preferred. Familiarity with OASIS assessments, Medicare Conditions of Participation for Home Health, and related regulatory requirements preferred. Ability to perform moderate physical activity including frequent walking, standing, bending, lifting up to 50 pounds, and driving for patient visits. Experience managing caseloads independently and documenting care accurately in clinical records preferred.
Assignment Details
Facility Type: Home Health Shift Name: Days Shift Details: Days Service Area: Clarendon, Sumter, Lee, Darlington, and Florence counties (service areas may require travel within these counties). Expectation to provide at least 20 in-home patient care units per week. Orientation: Agency allows up to two weeks of orientation pay with pay equivalent to 30 units. Limited requested time off; requests should not extend more than two consecutive days. Floating: May be required to travel within the listed service counties for patient visits.
