Minimum Requirements

• Doctoral, Master's, or Bachelor's Degree in Physical Therapy from an accredited program approved by the APTA.
• Active Ohio Physical Therapist license required to start the assignment.
• BLS certification from AHA or ARC may be required.
• Minimum 1 year of clinical experience in home health or hospice recommended; experience with home health patient evaluations, plan of care development, and skilled documentation preferred.
• Comfort with electronic medical records and productivity expectations. Client requires weekly productivity target of 30 points and a weekly hour guarantee of 36 hours.
• Valid driver's license and reliable transportation for patient visits. Mileage between visits will be reimbursed.
• Willingness to follow client dress code: gray or black scrubs.
• Preferred: prior experience working with LHC-style documentation or familiarity with LHC Group workflows. Preferred: hospice experience and skills in mobility, balance, and fall-prevention interventions.
