Lpn - Grand Junction, Co Mesa County, Co; Colorado

As an LPN with Stellar Senior Living, you will play a key role in delivering high-quality, compassionate care to residents in our senior living community. Working within our Healthcare & Medical Services department, you will provide hands-on nursing care, including assessing residents, monitoring vital signs, and supporting individualized care plans in collaboration with RNs, physicians, and other healthcare professionals. You will be responsible for safe and accurate medication administration, treatments, wound care, and documentation in accordance with state regulations, facility policies, and best practices. In this role, you will help coordinate resident care, respond promptly to changes in condition, and communicate clearly with residents, families, and the interdisciplinary team. You will supervise and support caregiving staff, offering guidance, training, and feedback to ensure high standards of care and a safe, respectful environment.
