About the Role

La Villa Grande Care Center in Grand Junction, CO, is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our Healthcare & Medical Services team. In this role, you will provide hands-on nursing care to residents in a skilled nursing environment, supporting their daily living needs while promoting comfort, safety, and dignity. Working under the direction of a Registered Nurse and in collaboration with an interdisciplinary care team, you will administer medications and treatments, monitor changes in condition, and ensure that each resident receives personalized, high-quality care. As an LPN at La Villa Grande Care Center, you will perform routine and complex nursing tasks, including vital sign monitoring, wound care, medication administration, and assistance with activities of daily living. You will accurately document assessments, care interventions, and resident responses in the electronic medical record, maintaining compliance with state and federal regulations as well as facility policies. You will actively participate in care planning by providing input on resident progress and communicating significant changes to the RN, physician, and family members. You will play a key role in fostering the warm, nurturing environment that defines La Villa Grande Care Center. The ideal candidate is a licensed LPN in the state of Colorado with solid clinical skills, the ability to manage multiple priorities, and a genuine passion for working with older adults and individuals requiring long-term or post-acute care. Experience in skilled nursing, long-term care, or a similar setting is preferred.
