Family Medicine, Virtual Primary Care Physician

Oak Street Health is seeking a Primary Care Physician to join our innovative Healthcare & Medical Services team. In this role, you will provide comprehensive, preventative primary care to an adult Medicare population in underserved communities. You’ll manage a smaller patient panel to allow for longer visits, focus on chronic disease management, and collaborate with an integrated care team including nurses, social workers, and care managers.
