Case Management Registered Nurse
About the Role
Mission Health, based in Asheville, NC, is seeking a compassionate and experienced Case Management Registered Nurse to join our Healthcare & Medical Services team. In this role, you will provide patient-centered, clinically sound case management for a diverse patient population, ensuring safe, efficient, and coordinated care across the continuum. Working within a collaborative environment that emphasizes teamwork, respect, and professional growth, you will serve as a key liaison among patients, families, physicians, and the interdisciplinary care team. The Case Management Registered Nurse is responsible for conducting comprehensive nursing assessments focused on clinical, psychosocial, and discharge needs, and for developing and implementing individualized care plans that promote high-quality outcomes. You will coordinate services from admission through post-discharge, facilitate safe transitions of care, and help reduce avoidable readmissions by proactively identifying and addressing barriers to recovery and ongoing care. Utilizing your clinical judgment, you will perform utilization review activities, help ensure appropriate level of care, and promote the effective use of hospital and community resources. In this position, you will provide patient and family education regarding disease processes, medications, follow-up care, and community resources, empowering patients to manage their own health as independently as possible. You will document all assessments, interventions, and communications within the Electronic Health Record, maintain awareness of payer requirements and regulatory standards, and contribute to quality and performance improvement initiatives related to case management and population health. This role requires strong communication, problem-solving, and organizational skills, as well as the ability to collaborate closely with physicians, social workers, therapists, and external providers. Qualifications
- RN license
- Case management experience
- Bachelors of Science in Nursing or equivalent
