About the Role
Mission Health, based in Asheville, NC, is seeking a dedicated Case Management Registered Nurse to join our Healthcare & Medical Services department. In this role, you will play a key part in ensuring patients receive coordinated, high-quality care throughout their healthcare journey. Working within a collaborative, patient-centered environment, you will use your clinical expertise and strong communication skills to assess patient needs, develop and manage individualized care plans, and coordinate services across the continuum of care.
The Case Management Registered Nurse will conduct comprehensive assessments to identify medical, psychosocial, and discharge planning needs, working closely with physicians, therapists, social workers, and other interdisciplinary team members. You will facilitate safe and timely transitions of care, including discharge planning, referrals to home health or rehabilitation services, and connections to community resources. Through proactive care coordination and patient advocacy, you will help reduce avoidable readmissions, support efficient utilization of hospital resources, and ensure that patients and families understand and can follow their plans of care.
In this position, you will perform utilization review activities, collaborate with payers to support appropriate levels of care, and ensure compliance with regulatory and organizational standards. You will maintain accurate, timely documentation in the Electronic Health Record (EHR), track patient progress, and adjust care plans as clinical needs evolve.
