Minimum 1 year of clinical experience in home health or hospice recommended; experience with home health patient evaluations, plan of care development, and skilled documentation preferred. Comfort with electronic medical records and productivity expectations. Client requires weekly productivity target of 30 points and a weekly hour guarantee of 36 hours. Preferred: hospice experience and skills in mobility, balance, and fall-prevention interventions.
