Physical Therapist Assistant (PTA) – Skilled Nursing Facility (SNF) 60;
We are seeking a dedicated and skilled 60;Physical Therapist Assistant (PTA) 60;to join a collaborative therapy team within a skilled nursing facility. This role focuses on providing high-quality rehabilitation services to short-term rehab patients and long-term residents, helping them regain mobility, independence, and confidence in daily activities. 60;
The PTA will implement individualized treatment plans developed by the supervising Physical Therapist, delivering skilled interventions that improve strength, balance, gait, and functional mobility. This position requires strong clinical judgment, communication skills, and the ability to manage a productive caseload in a supportive, team-based environment. 60;
Key Responsibilities: 60;
Provide skilled physical therapy interventions under the supervision of a licensed PT 60;
Assist patients with transfers, gait training, balance, strength, and endurance exercises 60;
Implement fall prevention strategies and assist with adaptive equipment recommendations 60;
Educate patients and caregivers on safe mobility and home exercise programs 60;
Monitor patient progress and communicate updates to supervising PT 60;
Accurately document treatment sessions, progress notes, and discharge summaries in EMR 60;
Collaborate with interdisciplinary team members including OT, SLP, nursing, and case management 60;
Maintain productivity while ensuring patient-centered care 60;
Ideal Candidate: 60;
Active PTA license in practicing state 60;
SNF or geriatric rehabilitation experience preferred 60;
Strong clinical reasoning and communication skills 60;
Comfortable managing both short-term rehab and long-term resident caseloads 60;
