• Valid and unrestricted Florida Physical Therapist license
• Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited program
• Current BLS certification through American Heart Association (AHA) or American Red Cross (ARC)
• Pre-employment health screenings including TB testing, immunizations (MMR, Varicella, Hepatitis B, Influenza when applicable)
• Florida ACHA fingerprint background check required prior to start date
• Skills checklist completed within one year and competency exam with 80% passing score
• Two professional references from the past three years, including one supervisory reference
