Now hiring a Physical Therapist (PT) for a 13-week Skilled Nursing Facility (SNF) assignment working day shifts from 8:00 AM – 4:30 PM with 35 guaranteed hours per week. The Physical Therapist will evaluate and treat patients with mobility, strength, balance, endurance, and functional limitations while developing individualized rehabilitation plans focused on restoring independence and improving quality of life in a skilled nursing environment. Candidates should be comfortable collaborating with interdisciplinary teams and documenting patient progress in accordance with clinical and regulatory standards.
Requirements:
Active Physical Therapist (PT) license required
BLS Certification required
SNF or rehabilitation experience preferred
Experience with geriatric patient populations preferred
Strong communication and documentation skills required
Ability to work independently and collaboratively within a rehab team
