Details- Home health and hospice setting providing in-home therapy services
- Monday through Friday schedule
- Up to 40 hours per week
- 13-week contract
- Caseload includes home care and hospice patients focused on independence and safety
- Active Vermont OT license required (license required if applicable)
- Valid driver's license and reliable personal vehicle required
- Home care setting experience preferred
- Ability to work independently while collaborating with a multidisciplinary team
- Provide in-home occupational therapy services aimed at maximizing patient independence and safety
- Collaborate as part of a multidisciplinary team serving home care and hospice patients
- DOB and SSN may be required for consideration
