Carle Health - - Responsibilities: Educates patients, family, caregivers, students, and other health care providers using relevant and effective teaching methods; Follows the therapist's plan of care and adjusts treatment based on patient response; Produces documentation in the daily notes; Provides treatment interventions including therapeutic exercise, gait and balance training, ultrasound; Coordinates with PT on patient treatment interventions and communicates with referring provider on patient progress
