Rn Nurse Manager Rehab and Long-term Care

LifeBridge Health is seeking an experienced RN Nurse Manager for our Rehab and Long-Term Care services within our Healthcare & Medical Services department. In this key leadership role, you will oversee the daily clinical and operational activities of our rehabilitation and long-term care nursing units, ensuring safe, high-quality, and compassionate care for a diverse patient population. You will lead a team of RNs, LPNs, CNAs, and other care providers, fostering a culture of collaboration, innovation, and community engagement that reflects LifeBridge Health’s mission and values.

As the RN Nurse Manager, you will be responsible for supervising and mentoring nursing staff, providing clinical guidance, and ensuring adherence to evidence-based practices and organizational policies. You will coordinate closely with an interdisciplinary team—including physicians, therapists, social workers, and case managers—to develop and implement comprehensive, patient-centered care plans that support rehabilitation goals, chronic disease management, and optimal long-term outcomes. Your role will include oversight of admissions, assessments, care planning, and discharge processes to support safe transitions of care.

You will play a central role in quality improvement by monitoring clinical outcomes, patient satisfaction, and safety metrics, identifying opportunities for improvement, and leading initiatives that enhance care delivery and operational efficiency. Ensuring regulatory and accreditation compliance, including CMS and Joint Commission standards, will be a critical component of your responsibilities. You will manage staffing, schedules, and resource allocation based on patient acuity and unit needs, while supporting staff education, professional development, and performance evaluations.
