About the Role

CarePartners is seeking a Licensed Practical Nurse - Home Health to provide high-quality, patient-centered care in the McDowell area. In this role, you will deliver skilled nursing services in patients’ homes, support care coordination, and collaborate closely with an interdisciplinary team to improve community health outcomes. Responsibilities include patient assessments, medication administration, care plan implementation, and patient/family education. The ideal candidate is compassionate, detail-oriented, and committed to collaborative, coordinated care that enhances access and quality across our provider network.

Responsibilities

Provide skilled nursing in patients' homes
Participate in care coordination with interdisciplinary team
Educate patients and families on care plans

Qualifications

LPN license
Home health/nursing experience
BLS/CPR or ACLS as applicable
