About the Role
CarePartners is seeking a compassionate Registered Nurse Hospice Homecare to deliver high-quality, patient-centered end-of-life care in patients’ homes. In this field-based role, you will perform comprehensive assessments, develop individualized care plans, and provide expert pain and symptom management. You will educate and support patients and families, coordinate services with an interdisciplinary hospice team, and ensure accurate, timely documentation. The ideal candidate is an experienced RN with strong clinical judgment, excellent communication skills, and a commitment to dignified, compassionate hospice care across our Connecticut community.
