About the Role
Tufts Medical Center is seeking a compassionate and skilled RN Case Manager, Hospice, Sign-on Bonus Eligible, to join our Healthcare & Medical Services team. In this role, you will provide comprehensive, patient-centered hospice case management to adult patients and their families, ensuring that each individual receives high-quality, dignified end-of-life care. As an essential member of our interdisciplinary hospice team, you will coordinate and deliver nursing care across settings, including the home and community, integrating evidence-based practice with the values of comfort, respect, and support. You will be responsible for conducting holistic nursing assessments, developing and updating individualized hospice plans of care, and collaborating closely with physicians, social workers, spiritual care providers, therapists, and other team members to optimize symptom management and quality of life. You will manage a defined caseload of hospice patients, prioritizing visits based on acuity and need, providing skilled nursing interventions, monitoring changes in condition, and responding promptly to urgent situations. Education and support for patients and their families will be central to your responsibilities, as you guide them through the physical, emotional, and psychosocial aspects of serious illness and the end-of-life process. Within this position, you will utilize your clinical judgment to manage pain and complex symptoms, administer medications and treatments according to established protocols, and ensure accurate, timely documentation in the electronic medical record. You will coordinate transitions of care, facilitate family meetings, and advocate for patient goals and preferences, while maintaining compliance with hospice regulations, Medicare and Medicaid requirements, and Tufts Medical Center’s clinical standards. Working in partnership with an experienced interdisciplinary team, you will participate in regular case conferences, contribute to quality improvement initiatives, and support continuous learning and best practices in hospice and palliative care.
