About the Role

CarePartners is seeking a compassionate and skilled Registered Nurse for our Inpatient Hospice Solace Team within the Healthcare & Medical Services department. In this role, you will provide high-quality, patient- and family-centered end-of-life care to individuals receiving inpatient hospice services, ensuring that each patient’s final days are met with dignity, comfort, and respect. As part of CarePartners, a Connecticut-based health organization dedicated to improving community health through coordinated, comprehensive care, you will work within a collaborative environment that values compassion, communication, and clinical excellence. The Registered Nurse Inpatient Hospice Solace Team nurse is responsible for comprehensive assessments, development and implementation of individualized care plans, and ongoing evaluation of patient responses to care. You will manage complex pain and symptom control, administer medications and treatments, and perform skilled nursing interventions consistent with hospice and palliative care standards. Working closely with an interdisciplinary team—including physicians, social workers, chaplains, and other clinical professionals—you will coordinate holistic care that addresses physical, emotional, and psychosocial needs. A key component of this position involves providing education and empathetic support to patients and their families, helping them understand disease progression, available comfort measures, and options for end-of-life decision-making. You will act as a clinical advocate, ensuring that patient wishes are honored and that communication between patients, families, and the care team is clear and compassionate. Accurate and timely documentation in the electronic health record, adherence to regulatory and organizational policies, and contribution to quality improvement and patient safety initiatives are essential responsibilities. The ideal candidate is an experienced hospice, palliative, or acute care nurse who thrives in a collaborative, mission-driven environment and is comfortable working in an inpatient setting with occasional coordination across care settings.
