Responsibilities
- Provide direct patient care within facility-based post-acute settings
- Collaborate with interdisciplinary team to develop and implement care plans
- Monitor and document patient status and outcomes
Qualifications
- Current RN licensure
- BLS/ACLS certifications preferred
- Experience in post-acute rehabilitation is a plus
