About the Role
CarePartners seeks a Registered Nurse to support coordinated, patient-centered care for our Connecticut members. In this role, you will collaborate with partnered providers to assess needs, create and manage care plans, and guide patients through the healthcare system. Responsibilities include care coordination, transitions-of-care support, and member education to improve outcomes and reduce avoidable utilization. You will document in electronic health records, track quality metrics, and contribute to population health initiatives. This hybrid position combines remote work with occasional on-site or field collaboration across our provider network.

Qualifications
- BSN or RN licensure
- CT state RN license
- Experience in care coordination preferred
