About the Role
CarePartners is seeking a compassionate Registered Nurse to join our PACE Program, supporting frail and elderly participants in a coordinated, community-based setting. In this role, you will provide direct nursing care, conduct assessments, develop and implement individualized care plans, and collaborate with an interdisciplinary team to optimize patient outcomes. You will educate participants and families, monitor chronic conditions, and ensure continuity of care across settings. Ideal candidates bring strong clinical judgment, excellent communication skills, and a passion for patient-centered, community-focused healthcare.
