About the Role
Hourly Pay Range: $38.67 - $59.94 - The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors.
Position: Performance Improvement Coordinator
Part Time: 28 hours per week
Hours: Monday-Friday, 8:00a-4:30p
Required Travel: Bachelor?s Degree or higher required, preferred in clinically-focused area such as RN, OT, PT, pharmacist, health information technology, or other healthcare-related degree with (MS preferred)
State of Illinois licensure in medically-related field (RN, PT, OT, SLP, pharmacist) or CPPS/CPHQ, LEAN/Six Sigma Certification and/or similar training
Minimum Work Experience
~ Minimum of 5 years of healthcare experience or formal training and/or certification in PI or team facilitation including leading projects or system improvement teams.
Analyzes Data Pertinent to Service Area to Identify Opportunities
Analyzes data pertinent to identify opportunities for improvement based on internal goals, external/internal benchmarking, industry standards and payor/external regulatory rating systems.
Interprets and reports data from multiple sources for comparison, benchmarking and performance assessment. Independently seeks out appropriate and timely benchmark data.
Monitors data for criteria that is selected to evaluate clinical care and patient safety
Able to develop reports from multiple sources to assure data is accurate and actionable for system improvement or reflective of issues being addressed
Serves as liaison between stakeholders (e.g., clinical leaders, physicians, payers and Health Information Technology), to assure accurate data elements for reporting are identified to maximize reporting effectiveness and utilization
Performance and Process Improvement Project Management
Manages performance and process improvement projects across the system
moves projects forward according to defined goals and timelines. Acts as a consultant with activities pertaining to various regulatory and/or accreditation requirements, encompassing tasks such as document provision, participation in discussions, and offering survey chaperone support.
~ Creates supportive tools as needed to monitor performance and develop proactive approached to enhance quality, safety, and outcomes. Maintains expert/leader level of competence in current Quality concepts, tools and performance measurements.
Serves as mentor/preceptor for new or less experienced staff in order to develop and maintain competence/expertise for other Quality team/organization staff members.
Consistently looks for and uses opportunities to network, compare outcomes and communicate both success and opportunities for improvement.
Effectively uses a wide variety of settings and formats to share outcomes, ideas and action plans including both formal and informal presentations, written materials, and media/tools (Power Point, bulletin boards, flyers, newsletters) to inform and educate staff, medical staff and community about local Quality achievements, initiatives and priorities.
