Minimum Requirements

  • Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
  • Current, unencumbered Oregon physical therapy license required to start the assignment.
  • Basic Life Support (BLS) certification required from AHA or ARC.
  • Minimum 2–3 years of physical therapy experience preferred. This is preferred, not required.
  • Experience providing care in home health settings preferred, including home visit evaluation and treatment planning.
  • Ability to supervise and evaluate Physical Therapy Assistants and Home Health Aides as assigned.
  • Familiarity with electronic documentation and basic computer skills including email, electronic health records, and online training platforms.
  • Must have reliable transportation, valid driver’s license, and agency-required liability insurance for home visits.


Assignment Details

  • Facility Type: Home Health.
  • Shift Name: Days.
  • Shift Pattern: 5x8.
  • Start Date Range: April 27, 2026 to July 26, 2026 (assignment dates provided by client).
  • Job Type: Travel, temporary help.
  • Clinical Responsibilities: Perform patient evaluations, provide therapeutic services per physician orders, update plans of care, document patient condition, instruct family and agency personnel, evaluate home environment, and participate in in-service education.
