Minimum Requirements
Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA. Active Illinois Physical Therapist license required to start the assignment. BLS certification from AHA or ARC may be required. Home Health experience preferred. Experience with OASIS and HCHB preferred. Ability to perform in-home physical therapy evaluations and manage patient care across multiple counties. Must be able to travel within coverage area counties: Clinton, Marion, Washington, Jefferson, Hamilton. Minimum education requirement: Bachelor's Degree.
Assignment Details
Facility type: Home Health. Shift Name: Days. Schedule: 4x8 (days). Weekly hour guarantee: 32 hours per week. On call/weekend requirement: No. Mileage reimbursement between patient visits from first to last visit: Yes. Coverage area includes multiple counties; candidate must be able to travel within the service area. Required certification: Illinois PT license.
