Physical Therapist - Travel Contract

Minimum Requirements

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Active Arizona Physical Therapist license required to start the assignment.
• BLS certification from American Heart Association or American Red Cross may be required.
• Experience in outpatient orthopedics preferred.
• Demonstrated competency in evaluation, plan of care development, goal writing, and documentation compliant with state and federal guidelines.
• Experience with functional outcome measures and letters of medical necessity preferred.
• Ability to supervise and delegate to therapy assistants and students as appropriate.
• Current professional liability coverage and ability to maintain required state continuing education and licensure in good standing.
