Minimum Requirements

• Bachelor of Science in Physical Therapy from an APTA-approved program or equivalent degree (required).
• Current Physical Therapist state license for Kentucky required to start the assignment.
• Proof of current CPR certification required.
• Current valid driver’s license, reliable transportation, and vehicle insurance required.
• 1-2 years of experience as a Physical Therapist in an acute care or rehabilitation setting required.
• Home health experience preferred.
• Ability to perform clinical assessments, develop and revise Plans of Care, document progress notes and phone orders, and collaborate with physicians and interdisciplinary teams (required).
• Ability to lift, bend, push, pull, and perform prolonged standing and walking as required for home visits (required).
• Ability to instruct patients and families on transfer techniques and use of durable medical equipment; knowledge of DME preferred.
• Meets mandatory continuing education requirements and demonstrates commitment to professional growth (required).
