Physical Therapist - Travel Contract

Minimum Requirements

  • Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program.
  • Active Wisconsin Physical Therapist license required to start the assignment.
  • Current BLS certification from AHA or ARC may be required.
  • Experience working in skilled nursing facility settings preferred.
  • Demonstrated ability to document skilled evaluations, plan of care, and skilled progress notes in an electronic medical record preferred.
  • Ability to work Monday through Friday schedule and meet a 35-hour weekly commitment as outlined by the client preferred.

  • Facility Type: Skilled Nursing Facility.

  • Work Days: Monday, Tuesday, Wednesday, Thursday, Friday.
  • Meal Break: 30 minutes.
  • Contract Length: 4 weeks.
  • Guaranteed Weekly Hours: 35.
