Physical Therapist - Travel Contract

Minimum Requirements

• Bachelor‘s, Master’s, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
• Active Indiana Physical Therapist license required prior to start of assignment.
• BLS certification from AHA or ARC may be required.
• Home health experience preferred. (Preferred)
• Experience with SOC, evaluation, and treatment in a home health setting preferred. (Preferred)
• Ability to document in electronic health record systems; knowledge of HCHB or PointClickCare is a plus. (Preferred)
• Compassion, interpersonal skills, flexibility, and problem-solving abilities preferred. (Preferred)
• Must be willing to travel within multiple counties for patient visits, including Adams, Allen, DeKalb, Huntington, Kosciusko, LaGrange, Noble, Steuben, Wells, and Whitley Counties.

Assignment Details

• Facility type: Home Health.
• Shift: Days.
• Shift hours: 08:00–16:00 (days).
• Weekly hour expectation: 36 hours guaranteed.
• Coverage area: Multiple counties across northeastern Indiana; mileage reimbursed between visits.
• On-call/weekend requirement: None.
