Minimum Requirements
- Associate, Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program.
- Active Florida Physical Therapist license required prior to the start of the assignment.
- OASIS proficiency required. Must perform documentation and outcomes per agency OASIS expectations (task volume noted as six visits per day in client requirements).
- Experience providing home health physical therapy, including travel to patients' homes across a multi-city service area (Gulf Breeze to Destin region).
- BLS certification from AHA or ARC may be required.
- Ability to travel daily to patient homes and manage a mobile caseload.
- Preferred: Florida-licensed Physical Therapist noted as a desirable attribute by the client.
- Preferred: Experience with electronic documentation systems used in home health agencies.
Assignment Details

- Shift hours: .
- Workdays: Monday through Friday as noted in client order details.
- Caseload expectation: Perform approximately six patient visits per day per client guidance.
- Daily travel required to patient homes across the service area. Candidate should be prepared to travel to multiple patient locations within a shift.
- Dress code: royal blue scrubs or dockers with polo-style shirt.
- Interview options: Teams or face-to-face at the agency.
