Minimum Requirements
Graduate of an accredited Physical Therapy program (Doctor of Physical Therapy or equivalent) Current, active, and unrestricted Physical Therapist license in Illinois Minimum of 1 year of clinical experience; home health experience strongly preferred Knowledge of home health regulations, including Medicare guidelines and documentation standards (OASIS experience preferred) Ability to perform comprehensive patient assessments and develop individualized plans of care Current CPR/BLS certification (American Heart Association or equivalent) Valid driver’s license, reliable transportation, and willingness to travel within assigned territory Ability to work independently in a home-based setting and manage a flexible schedule Strong clinical decision-making, communication, and documentation skills Preferred: Experience with electronic medical records (point-of-care EMR), geriatrics, chronic disease management, bilingual skills, or prior home health/community-based care
