About the Role
Eastside Rehabilitation Hospital is seeking a dedicated Physical Therapist to join our Healthcare & Medical Services team and support our mission of helping patients regain mobility and improve their quality of life. As a Physical Therapist in our specialized rehabilitation hospital, you will provide advanced therapeutic services to a diverse patient population recovering from orthopedic, neurological, post-surgical, and medically complex conditions. You will perform comprehensive evaluations, develop individualized, evidence-based treatment plans, and deliver hands-on care designed to restore function, reduce pain, and maximize independence.
In this role, you will utilize a wide range of therapeutic interventions, including therapeutic exercise, manual therapy, gait and balance training, neuromuscular re-education, and appropriate use of modalities and assistive devices. You will work in close collaboration with an interdisciplinary team of physicians, nurses, occupational therapists, speech therapists, and case managers to coordinate patient-centered care across the continuum of rehabilitation. Daily responsibilities include accurate and timely documentation in the electronic medical record, ongoing assessment of patient progress, and adjustment of treatment plans based on clinical outcomes and patient goals.

You will play a key role in preparing patients for safe discharge by assessing functional status and recommending appropriate equipment, supports, and follow-up services.
