About the Role

Stellar Senior Living is seeking a compassionate and motivated Physical Therapist (PT) to join our Healthcare & Medical Services team, supporting residents in our vibrant senior living communities. In this role, you will provide high-quality, person-centered physical therapy services that enhance mobility, independence, and quality of life for seniors in independent living, assisted living, and memory care settings.
As a Physical Therapist with Stellar Senior Living, you will perform comprehensive evaluations, develop individualized treatment plans, and deliver evidence-based interventions to address strength, balance, gait, pain management, and functional mobility. You will collaborate closely with nursing staff, physicians, caregivers, and other members of the interdisciplinary team to ensure that each resident receives coordinated, holistic care tailored to their unique goals and medical needs.
Your day-to-day work will include leading therapeutic exercises, balance and fall-prevention programs, transfer and gait training, and endurance activities designed to support residents in their daily routines. You will educate residents and families on home exercise programs, proper body mechanics, safe use of assistive devices, and strategies to maintain independence. Accurate and timely documentation of assessments, treatment sessions, and progress notes will be essential to support regulatory compliance and track clinical outcomes.

You will contribute to care conferences, adjust treatment plans as residents’ needs change, and help mentor therapy assistants and aides when appropriate.
