Certifications: Licensed Physical Therapist (PT) - Illinois Department of Financial and Professional Regulation (IDFPR); Bachelor's Degree: Physical Therapy (Required), Work Experience: Reviews requisitions for physical therapy and evaluates patient's physical condition and needs.Establishes PT treatment goals for each individual patient, based on the findings of the initial proper equipment usage, instructs the patient in appropriate program, and modifies program as needed.Teaches patient, other staff members, and student affiliates in passive, active assistive, or resisted exercise, ambulation, and other physical movements in order to articulate and demonstrate principles of rehabilitative therapy.Evaluates clinical findings to determine physical therapy diagnoses and outcomes of care.Documents as required by the profession and department, such as evaluation results, treatment plans, progress reports, and other such reports.Collaborates with support personnel according to legal standards and ethical guidelines.Establishes a PT Plan of Care based on functional deficits and patient goals. Completes objective testing for range of motion, strength, and performance of daily functional tasks. Completes therapeutic modalities, as appropriate, including ultrasound, iontophoresis, and electrical stimulation. Completes daily documentation and charging in Therapy EMR. Provides supervision to Physical Therapy Assistants, in accordance with regulatory agency guidelines. Utilizes standardized outcome measures in a uniform manner to ensure functional performance is accurately reflected.
