Physical Therapist

Mohawk Valley Health System (MVHS), a leading provider of comprehensive healthcare services in Upstate New York, is seeking a compassionate and patient-focused Physical Therapist to join our Healthcare & Medical Services team. In this role, you will evaluate, plan, and implement evidence-based physical therapy interventions to help patients restore function, improve mobility, reduce pain, and prevent or limit physical disabilities. Working collaboratively with physicians, nurses, and other members of the interdisciplinary care team, you will perform detailed assessments, develop individualized treatment plans, set measurable functional goals, and provide hands-on therapeutic interventions tailored to each patient’s needs. You will educate patients and families on home exercise programs, safe mobility techniques, and strategies to support long-term recovery and independence. You will document all evaluations, treatments, and progress notes accurately and timely within our electronic medical record system, ensuring continuity and quality of care across the care continuum.
