Physical Therapist, Home Health

CenterWell Home Health is seeking a compassionate and skilled Physical Therapist for in-home patient care within the Healthcare & Medical Services department. In this role, you will travel to patients’ homes to provide individualized physical therapy services that restore function, improve mobility, and enhance quality of life. Working closely with nurses, occupational therapists, physicians, and caregivers, you will perform comprehensive evaluations, develop evidence-based treatment plans, and deliver hands-on therapeutic interventions tailored to each patient’s goals and home environment. You will educate patients and families on home exercise programs, safe mobility techniques, fall prevention, and use of assistive devices, empowering them to maintain independence. Documentation of assessments, treatment sessions, progress notes, and care plans will be completed accurately and promptly in the electronic medical record to support continuity of care and regulatory compliance.
