Physical Therapist, Home Health

CenterWell Home Health is seeking a compassionate and motivated Physical Therapist, Home Health to provide high-quality, individualized rehabilitation services to patients in the comfort of their own homes. In this role, you will help patients regain mobility, improve function, and enhance independence by performing comprehensive evaluations, developing evidence-based treatment plans, and delivering skilled therapeutic interventions tailored to each patient’s goals and home environment. You will collaborate closely with nurses, occupational therapists, speech therapists, physicians, and caregivers to ensure coordinated, patient-centered care and optimal outcomes. Key duties include conducting initial and ongoing assessments, setting functional and measurable goals, administering therapeutic exercises and manual therapy techniques, training patients and families on safe mobility and use of assistive devices, and regularly reassessing progress to update care plans. You will document all visits, interventions, and patient responses accurately and timely in the electronic medical record, ensuring compliance with regulatory and payer guidelines. You will manage a caseload within a defined geographic territory, plan efficient routes, and communicate schedule changes promptly.
