About the Role
CenterWell Home Health is seeking a compassionate and skilled Physical Therapist for in-home care, providing high-quality therapeutic services to patients in the comfort of their own residences. As a Physical Therapist, Home Health, you will conduct comprehensive evaluations, develop individualized treatment plans, and deliver evidence-based interventions designed to restore function, reduce pain, and enhance patient independence and quality of life. Working within the Healthcare & Medical Services department, you will collaborate closely with nurses, occupational therapists, speech therapists, physicians, social workers, and family caregivers to ensure coordinated, patient-centered care.
In this role, you will travel to patient homes each day, assessing home safety, recommending adaptive equipment, and educating patients and caregivers on exercises, mobility strategies, and fall prevention techniques. You will monitor progress, adjust care plans as needed, and meticulously document all assessments, treatments, and outcomes in accordance with regulatory and CenterWell standards. You will be responsible for managing your caseload efficiently, prioritizing visits, and communicating changes in patient condition promptly to the interdisciplinary team.
