Physical Therapist -greater New London, Ct

FOX Rehabilitation is seeking a compassionate, clinically skilled Physical Therapist to join our clinician-owned and operated private practice in the Greater New London, CT area. As a FOX Physical Therapist, you will specialize in geriatric rehabilitation, providing evidence-based, patient-centered care designed to improve function, promote safety, and enhance quality of life for older adults across a variety of community and on-site settings.
In this role, you will independently evaluate older adult clients, identify functional limitations, and develop individualized, goal-oriented treatment plans using current best practices in geriatric physical therapy. You will deliver interventions focused on strength, balance, gait, endurance, and functional mobility, with a strong emphasis on fall prevention, safe transfers, and maximizing independence in the home and community. You will regularly reassess progress, adjust plans of care, and communicate outcomes clearly to patients, families, and referring providers.
Working within FOX Rehabilitation’s culture of respect, professional growth, and clinical excellence, you will collaborate closely with an interdisciplinary team—including occupational therapists, speech-language pathologists, physicians, and other healthcare professionals—to coordinate comprehensive care. You will provide ongoing education and training to patients, caregivers, and family members on home exercise programs, safe mobility strategies, and use of assistive devices.
You will be responsible for accurate, timely documentation of evaluations, treatments, progress notes, and discharge summaries in an electronic medical record system, in accordance with Medicare and other payor guidelines as well as practice policies. You will manage your daily schedule and caseload efficiently, balancing productivity expectations with a strong commitment to high-quality, ethical care.
