Physical Therapist Assistant Home Visit

CenterWell Home Health is seeking a dedicated Physical Therapist Assistant for home visits to support patients in regaining mobility, strength, and independence in the comfort of their own homes. As part of a Healthcare & Medical Services team committed to compassion, integrity, and excellence, you will deliver hands-on therapeutic care while building meaningful relationships with patients and their families. Working under the direction and supervision of a licensed Physical Therapist, you will carry out treatment plans tailored to each patient’s unique needs, focusing on functional improvement, safety, and quality of life. In this role, you will travel to patient residences within an assigned area, provide therapeutic exercises and gait training, monitor patient progress, and document clinical findings using an electronic medical record system. You will educate patients and caregivers on home exercise programs, safe mobility techniques, fall prevention strategies, and proper use of assistive devices to promote autonomy between visits. You will collaborate closely with nurses and other therapy professionals to coordinate care, communicate changes in patient condition, and contribute to goal setting and plan-of-care updates.
