About the Role
Medical University of South Carolina - - Responsibilities: Assist the patient to better direct their own health care experience using screening and reinforcement of the treatment plan, educational programs, and linkage to health services and community resources.; Serve as a patient liaison to assist patients and caregivers in scheduling within the complex healthcare system.; Act as an advocate for an individual's health care needs by identifying and communicating opportunities for care intervention that have been established by the RN Navigator or Coordinator, including identifying and addressing gaps in care.; Provide timely access to quality patient care while serving as a vital link in communication between multidisciplinary providers internal and external to MUSC during initial treatment planning.

Qualifications
- Bachelor's degree in a health-related field preferred?
- Experience in patient navigation or care coordination recommended?
