About the Role

As an Occupational Therapist, Home Health with CenterWell Home Health, you will deliver personalized, in‑home therapeutic services to adult and geriatric patients in the community. CenterWell Home Health provides at‑home healthcare focused on enhancing patient autonomy and quality of life, and you will play a key role in helping patients safely remain in their homes by restoring function, promoting independence, and preventing decline. In this role, you will complete comprehensive occupational therapy evaluations in the patient’s home, identifying physical, cognitive, and environmental barriers to safe daily living. You will develop and implement individualized, evidence‑based treatment plans that address activities of daily living, home safety, functional mobility, upper extremity strength and coordination, adaptive equipment needs, and caregiver training. You will provide hands‑on therapy, patient and family education, and home exercise programs tailored to each patient’s goals, while closely monitoring progress and adjusting interventions as needed. Working within the Healthcare & Medical Services team, you will collaborate with nurses, physical and speech therapists, social workers, physicians, and caregivers to ensure coordinated, high‑quality care. Accurate, timely documentation in the electronic medical record and adherence to regulatory, payer, and CenterWell clinical standards are essential. You will prioritize patient safety, infection control, and risk reduction during all home visits, and communicate changes in condition promptly to the care team.
