Medical Social Worker

Satellite Healthcare is seeking a compassionate Medical Social Worker to support individuals living with chronic kidney disease in our Healthcare & Medical Services department. In this role, you will provide psychosocial assessments, counseling, and care coordination for patients receiving in-center and home dialysis. You will collaborate closely with nephrologists, nurses, dietitians, and other team members to address emotional, social, and resource needs.
