Mds Registered Nurse

Franciscan Ministries is seeking an MDS Registered Nurse to coordinate and oversee the Minimum Data Set (MDS) and care planning process within our Healthcare & Medical Services department. In this role, you will complete and audit MDS assessments, ensure accurate RUG/PDPM classifications, and collaborate with interdisciplinary teams to develop individualized care plans that reflect residents’ needs and goals. You will educate staff on documentation standards, monitor regulatory compliance, and support quality improvement initiatives.
