Job Title: Licensed Master Social Worker (LMSW)Location: Blue Springs, MissouriSalary: $65,000-$72,500Shift: Full-time, Monday through Friday, 8:00 AM to 5:00 PMJob Overview:We are seeking a Licensed Master Social Worker (LMSW) to join its Home Health and Hospice team serving both Kansas and Missouri locations. This is a full-time, flexible role focused on providing hospice and home health social work services to adult and geriatric patients in their homes. The LMSW plays a critical role in supporting patients and families through psychosocial assessments, care planning, education, and emotional support while collaborating closely with interdisciplinary care teams.
