RN Manager Home Health - Providence Home Health Services Oregon Schedule: Full Time/Days Supports Home Health East Portland team Provides oversight of all patient care services and personnel (interdisciplinary team). Making patient and personnel assignments, coordinating patient care, coordinating referrals, ensuring patient needs are continually assessed, ensuring development, implementation, and updates of the individualized plan of care. Review of required documentation, facilitates interaction between field staff, office staff, the home health patients, and their providers. Bachelor’s Degree in respective Clinical Specialty. Oregon Registered Nurse License upon hire. EMR experience (EPIC, Homecare Homebase) 3 years of Home Health Experience.
