Good Samaritan Health Center is seeking a dedicated LPN FT Day to join our Healthcare & Medical Services team providing compassionate, holistic care to underserved communities. As a full-time day-shift Licensed Practical Nurse, you will work in a collaborative, mission-driven environment focused on wellness, integrity, and community engagement. You will provide direct patient care under the supervision of registered nurses and providers, including performing assessments within LPN scope, obtaining vital signs, administering medications and injections, providing wound care, assisting with procedures, and monitoring patient responses to treatment. You will support care coordination, patient education, and chronic disease management, ensuring that patients and families understand care plans, medications, and follow-up instructions. In this role, you will document all care accurately and timely in the electronic health record, follow infection prevention and safety protocols, and participate in quality and performance improvement initiatives aimed at enhancing patient outcomes. You will collaborate closely with physicians, nurse practitioners, RNs, medical assistants, social workers, and other team members to address medical, social, and behavioral health needs, helping patients navigate community resources and supportive services.
