About the Role

Good Samaritan Health Center is seeking a compassionate LPN FT Day to join our Healthcare & Medical Services team, serving patients in underserved communities with a focus on holistic health and wellness outcomes. As a full-time day-shift Licensed Practical Nurse, you will provide direct patient care under the supervision of registered nurses and providers, supporting assessments, administering medications, performing treatments, and monitoring patient status while documenting all care accurately in the electronic health record. You will coordinate with an interdisciplinary team to support care plans, educate patients and families on health conditions, medications, and self-care, and help connect patients with community resources to address social determinants of health. In this mission-driven role, you will embody compassion, integrity, and community engagement by building trusting relationships, advocating for patients, and ensuring a respectful, culturally sensitive care environment. Responsibilities include obtaining vital signs, performing point-of-care testing, assisting with procedures, managing phone triage and patient messages as appropriate to scope, and ensuring infection control and safety standards are consistently followed. You will participate in quality improvement initiatives, contribute to population health and preventative care efforts such as immunizations and chronic disease management, and support clinic flow through efficient rooming, scheduling support, and coordination of follow-up care.
