La Villa Grande Care Center in Grand Junction, CO, is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our Healthcare & Medical Services team. In this role, you will provide direct hands-on nursing care, including assisting residents with daily living activities, personal hygiene, mobility, and comfort measures. You will safely administer medications and treatments in accordance with physician orders and within the LPN scope of practice, and you will carefully monitor residents’ conditions, recording and reporting any changes to the Registered Nurse and provider team. You will support the development and implementation of individualized care plans, perform basic wound care and other nursing procedures according to your licensure, and help ensure that every resident maintains dignity, respect, and as much independence as possible.

The LPN will collaborate closely with RNs, CNAs, therapists, physicians, and other members of the interdisciplinary healthcare team to coordinate and optimize resident care. You will communicate clearly and compassionately with residents and their families, offering education, updates, and emotional support as needed. A strong focus on safety, infection prevention, fall prevention, and adherence to all regulatory and facility policies is essential. You will participate in admissions, transfers, and discharges, ensuring accurate documentation and smooth transitions of care, while also contributing to a positive, team-oriented work environment.
