About the Role
La Villa Grande Care Center in Grand Junction, CO, is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our Healthcare & Medical Services team. In this role, you will provide hands-on nursing care to residents in a skilled nursing and long-term care setting under the direction of a Registered Nurse and within the scope of LPN practice. You will administer medications and treatments safely and accurately, monitor residents’ conditions, and promptly report significant changes to the RN or provider. Daily responsibilities include performing clinical tasks such as wound care, dressing changes, vital signs monitoring, tracheostomy and oxygen management as appropriate, catheter care, and assistance with G-tube feedings when required by resident care plans.
You will be responsible for timely and thorough documentation in the electronic health record, ensuring that care, observations, and resident responses are clearly and accurately recorded. Working closely with RNs, physicians, certified nursing assistants, therapists, and other members of the interdisciplinary team, you will help develop, implement, and adjust individualized care plans that align with residents’ goals, preferences, and medical needs. You will participate in admission, transfer, and discharge processes, helping to coordinate care and communicate effectively with residents and families.
Beyond clinical skills, this position requires a strong commitment to compassionate service and resident advocacy. You will support residents with activities of daily living as needed, promote safety and fall prevention, and foster an environment that encourages dignity, independence, and engagement. Clear, respectful communication with residents, families, and team members is essential, as you will often provide education about medications, treatments, and care routines, as well as emotional support during times of change or stress.
As a member of La Villa Grande Care Center, you will contribute to our supportive and engaging workplace culture by modeling professionalism, empathy, and teamwork. You will follow all infection prevention and control guidelines, adhere to state and federal regulations for long-term care, and participate in quality improvement initiatives, in-service education, and ongoing professional development. You may assist with orienting new staff and supporting CNAs, helping to ensure consistent, high-quality care across all shifts.
This is a full-time, on-site position in a clinical care environment that may involve rotating shifts, including evenings, nights, weekends, and holidays, to meet the needs of our residents.
